AGING AND DISABILITY RESOURCE CENTER: Provides assistance to individuals who are seeking long-term support information and counseling regarding available long-term support options, and determining eligibility for public long-term support programs and benefits. Benefits screening includes level of care determinations for Medicaid home and community-based service waiver programs. Offers in-person and over the phone assistance to individuals, families, and caregivers and referrals to programs/benefits (such as health education, nutrition, income support programs and more). *** Also assists older adults and adults with disabilities apply for the Homeowners' and Renters' Tax Credit program. The gross household income cannot exceed $60,000. ***
